Maybe "Escape from Innsmouth"? (1st printing in 1992, revised in 1997) Unfortunately it seems to be out of print, and fetches quite high prices on eBay and others. You can get the first edition for less than $50 from Amazon (link (http://www.amazon.com/Escape-Innsmouth-Cthulhu-Horror-Roleplaying/dp/0933635656)). The second is potentially more expensive. Try browsing around. Maybe there's a bargain somewhere.

Then there are Innsmouth-related story collections still in stock at Chaosium.

Yes, it looks like escape from Innsmouth maybe the source book I was looking for.
Unless I was dreaming when I held it in my hands at Forbidden Planet or Orcs Nest ( not that impossible lol) then it has possibly been reprinted
The review on Yog Soggoth.com ( a very usefull website for all HPL fans) makes it sound very valuable for anyone interested in Innsmouth as a setting
I will try to lay my hands on a copy as soon as possible

Barty you old crumpet dodger.....
Have you tried wargamesvault.com or DriveThruRPG.com  ??? They may have a PDF copy to download for a lot less. YES,yes, I understand that you then have the hassle of printing it out...but at least you will have a copy.
