Need Help Identifying a Figure

I found this figure in a box of mine mixed with a large variety of old Hinchcliffe, Ral Partha, Rafm and Grenadier figures.   As best I can tell it's a frog/lizard guy carrying a beholder cthulu mutation.  Any help figuring out the manufacturer and name is appreciated:

It was part of the Frother's Unite 2010 sculpting competition, Miniatures of Madness. I have one that I got when I bought the set but because their forum has recently been restarted I can't find any more details

Here's the one I painted. He was used in a game once and got dubbed Derek for some strange reason  ;D
