It's from Gale force 9 I believe but all the D&D minis they do are limited edition or something and I don't think they have it any more.

The Big Worm is a BEAUTY...in his/her/whatever own way. Your choice of colors & brushwork are OUTSTANDING. The base is FANTASTIC; I love the way he/she/whatever is erupting from the ground...very dramatic. VERY WELL DONE!
