I received a copy of this excellent game from On Military Matters in US.  Unfortunately, it did not come with the separate playsheet that's supposed to be included.  I don't suppose anyone has a scan available?

Well, that shouldn't have happened.  i will try and photograph the revised rules one this weekend and post.
